Speak without fear.

One of the biggest problems most people face when learning a foreign language is their own fear. They are worry about making mistakes and how well they're getting their words come out. None learn to speak something without failing at the beginning. We're so afraid to look stupid that we keep shut. The faster way to learn English is basically putting it into practice. A lot of practice is required and as you know, speaking to people, if you can do so, is the way you should go on.

My accent is so rusty and ridiculous.

Who cares? We usually are victims of badly intentioned people who make fun of the accent just because they think they do that much better than you do. Maybe it's true, maybe it's not. You should focus on being understood. If you get your message across, the first wall built up as obstacle will fall down on its basis.
There's a kind of paradigm regarding the accent. Some think it's ridiculous to have a heavy accent. For most americans accents in general are romantic and hold a special feature that says things about your background and origin. If I were you, I'd prefer the one that's good. Be positive and everything will go according to plan.

I Panic when the time to talk comes.

Though you think you're the only one that experiences these kind of dissapointing situations, there are a lot of people that think likewise. Avoid hesitating, get focused, don't stop believing you can do it as well as the speaker can do and relax. Nobody is going to kill you in case you make a mistake.
Sometimes, we English students believe that just for the reason that we're studying English teaching at university we must be experts at what we do and barely make no mistakes. This is a falace and it being part of a falace means to lie to ourselves. We're studying owing to our need to learn the stuff we don't know and to ask the teachers as many times as we require to reach this goal.

Sorround yourself with English.

The title explain by itself. Get a radio and listen to English speaking music, be fan of some artists and practice as much as you can so that your ears will get used to that.
Read articles about your favorite actors and sportmen and make English language your men-made second "main" language. You'll notice that this need for learning English goes with a lot of enthusiasm and willing to always want to learn more. It works out fine!
Try to find people whose first language's English and speak to them without a lick of fear. Never get frustrated if you're not understood because you're still learning. I strongly recommend that you should always let people correct your mistakes.
Mistakes are not errors!

Do exercises and take test often.

Regular studying is always good to solve some of the doubts you have when the teacher isn't around.

Record your voice yourself.

If you have a recorder, it would be a great idea to record yourself. Nobody likes to hear their own voice on tapes but likes tests. It's good to compare your tapes from time to time. You may be impressed with the progress you're making thay you don't mind your voice as much.
